چکیده

HNF-1b mutations. Most importantly, patients with an HNF-1b mutation may also be at risk of developing renal cell carcinomas, since recent evidence suggests that HNF-1b is a tumour suppressor gene [5]. In conclusion, patients with unexplained nephropathy and/or renal cysts should be routinely screened for an HNF-1b mutation in order to differentiate the RCAD syndrome from diabetic nephropathy. Ultimately, these patients are good candidates for combined kidney and pancreas transplantation.

Recent advances in disseminated intravascular coagulation: endothelial cells and fibrinolysis in sepsis-induced DIC

Endothelial cells are highly active, sensing and responding to signals from extracellular environments. They act as gatekeepers, mediating the recruitment and extravasation of proinflammatory leukocytes to the sites of tissue damage or infection. Endothelial cells participate in fibrinolysis by secreting tissue-type plasminogen activator, which converts plasminogen to active enzyme plasmin thro...

Early venovenous haemodiafiltration for sepsis-related multiple organ failure

In a prospective observational study that included 60 consecutive patients over a 10-year period, Page and coworkers [1] studied the effects of early continuous venovenous haemodiafiltration (CVVHDF) during sepsis-induced multiple organ failure. In two-thirds of the patients rapid metabolic improvement during CVVHDF was associated with circulatory improvement and a low mortality rate, whereas l...

Complement inhibition decreases the procoagulant response and confers organ protection in a baboon model of Escherichia coli sepsis.

Severe sepsis leads to massive activation of coagulation and complement cascades that could contribute to multiple organ failure and death. To investigate the role of the complement and its crosstalk with the hemostatic system in the pathophysiology and therapeutics of sepsis, we have used a potent inhibitor (compstatin) administered early or late after Escherichia coli challenge in a baboon mo...

The compensatory anti-inflammatory cytokine interleukin 10 response in pediatric sepsis-induced multiple organ failure.

STUDY OBJECTIVES To determine the circulating anti-inflammatory cytokine interleukin 10 (IL-10) response during the development of sepsis-induced multiple organ failure in children. DESIGN Prospective study. SETTING University pediatric ICU. PATIENTS Fifty-three consecutive children with sepsis and 15 critically ill children without sepsis. INTERVENTIONS Plasma IL-10, interleukin 6 (IL-...

Adrenal failure in the critically ill.

Three critically ill patients suffered multiple organ failure secondary to sepsis. Despite adequate supportive therapy and appropriate antibiotic cover, they failed to improve and required inappropriate inotrope support. They had not been treated with steroids or other drugs known to suppress adrenal function. Adrenal insufficiency was suspected. A random cortisol concentration and a short syna...
